主观用力呼吸困难与限制慢性阻塞性肺疾病患者运动的标准的关系

Relationship Between Exertional Dyspnea and Limitation for Patients With COPD to Exercise

目的 :探讨主观用力呼吸困难不宜作为限制慢性阻塞性肺疾病 (COPD)患者运动的标准。方法 :选择稳定期COPD患者 2 6例 ,进行症状限制性心肺运动负荷试验。运动终止后立即用血气分析仪监测此状态下的动脉血氧分压 (PaO2 max)。BorgScale(BS)被用于判定患者运动时主观出现用力呼吸困难的程度。结果 :BS与PaO2 max负相关 (r=- 0 .6 73,P<0 .0 1) ,15例患者因感到呼吸困难而终止试验 ,其中 9例发生血氧分压下降 ,但只有 4例PaO2 max <6 0mmHg。 2、BS与 ΔPaO2 负相关 (r=- 0 .4 4 ,P <0 .0 5 ) ,即BS与患者运动时发生的氧分压降成正相关。结论

Objective: Our aim was to prove that exertional dyspnea is not a limitation for patients with chronic obstructive pulmonary disease (COPD) to exercise. Methods: Maximum ergometer symptom-limited exercise (MESLE) were performed in 26 patients with COPD. Borg Scale (BS) was used for counting degree of exertional dyspnea which appeared during exercise and PaO 2 was measured at maximum exercise (PaO 2max). Results: There was a negative correlation between BS and PaO 2max (r=-0.673,P<0.01). Nine patients appeared desaturation, and only 4 patients had lower PaO 2max than 60mmHg at MSELE among 15 patients who couldnt exercise because of dyspnea. Conclusion: Exertional dyspnea could not be regarded as a limitation for patients with COPD to exercise.
