大肠癌组织中hMLH1的表达及其临床意义 被引量:3

摘要 目的 了解错配修复蛋白hMLH1在大肠癌组织中的表达及其与大肠癌发生、组织分化、淋巴结转移及临床分期的关系。方法 应用SP免疫组织化学法对 65例大肠癌组织病理切片进行免疫组化分析。结果 大肠癌旁正常组织hMLH1的表达阳性率为 80 % ,明显高于癌组织hMLH1的表达阳性率 49.2 % ( 0 .0 1<P <0 .0 5 ) ;高分化腺癌、中分化腺癌和低分化腺癌组织中hMLH1的表达率分别为 68.2 %、44 .1%和 2 2 .2 % ,高分化腺癌与低分化腺癌之间hMLH1的表达存在显著性差异 ( 0 .0 1<P <0 .0 5 ) ,但高分化腺癌与中分化腺癌、中分化腺癌与低分化腺癌之间hMLH1的表达无显著性差异 (P >0 .0 5 )。伴有淋巴结转移者大肠癌组织中hMLH1表达的阳性率 ( 2 3 .5 % )低于无淋巴结转移者 ( 5 8.3 % ) ( 0 .0 1<P <0 .0 5 ) ;临床Ⅲ~Ⅳ期大肠癌组织中hMLH1表达的阳性率 ( 2 6.3 % )低于临床Ⅰ~Ⅱ期 ( 5 8.7% ) ( 0 .0 1<P <0 .0 5 )。结论 hMLH1表达水平与大肠癌的发生、组织分化。 Objective To investigate the expression of DNA mismatch repair protein hMLH1 in colorectal carcinoma tinssue and its relationship to the occurrence of colorectal carcinoma,histodifferentiation,lymph nodes metastasis,and clinical development of the case.Methods The expression of hMLH1 in colorectal carcinoma tissue of sixty-five cases was analysed by SP immunohistochemical method.Results The incidence (80%) of expression of hMLH1 in the normal tissue around tumor tissue is higher than that (49.2%) in tumor tissue (0.01<P<0.05).The incidences of hMLH1 expression in high grade,moderate grade,low grade adenocarcinoma are 68.2%,44.1%,22.2% respectively.The incidence is significantly different between high grade and low grade adenocarcinoma (P<0.05).No significant difference of the hMLH1 expression incidence was found between high grade and moderate grade adenocarcinoma and between moderate grade and low grade adenocarcinoma.In cases with lymph nodes metastasis,the hMLH1 positive rate is 23.5%,lower than that in cases with no lymph nodes metastasis (58.3%) (0.01<P<0.05).The hMLH1 positive rate in the colorectal carcinoma tissue in clinical stages Ⅲ and/or Ⅳ was (26.3%),lower than that clinical stages Ⅰ and/or Ⅱ (58.7%) (0.01<P<0.05).Conclusions There is a correlation between the occurrence of colorectal carcinoma,histodifferentiation,lymph nodes metastasis,and clinical development of the case.
