应用国产带锁髓内针治疗胫骨骨折

Fractures of the Tibia Treated with homemade intramedullary interlocking Nai.

目的 探讨国产带锁髓内针对胫骨干骨折治疗的效果。方法 作者回顾 60例胫骨骨折 ,均采用国产带锁髓内针治疗。其中闭合性骨折 2 8例 ,开放性骨折 3 2例。除 8例失访外 ,其余 5 2例平均随访 18个月 ( 8-2 8个月 )。结果 最终结果评定采用Jonher-Wruh评分标准 ,优 40例 ,良 6例 ,中 6例 ,差 0例。骨折平均愈合时间 4个月。有 2例出现近端锁钉断裂 ,但未影响骨折愈合。无髓内针弯曲及断裂 ,无神经血管损伤 ,未出现小腿骨筋膜室综合征和脂肪栓塞综合征 ,无术后感染。结论 国产带锁髓内针适用于胫骨干骨折的治疗且应用远端锁钉瞄准装置 ,省时有效。

Objective To analyse and evaluate the efficiency of intramedullary interlocking nail (homemade product) in the treatment of Tibia shaft fractures.Methods Sixty fructures of the Tibia shaft treated with intramedullary interlocking nail were reviewed.There were 28 closde and 32 open fractures.Results Fifty-two patients were followed-up for an average of 18 months (rangde 8 to 28 months) and evaluated according to the criteria of johner and wrun.There were 40 assessed as excellent,6 as good and 0 as poor.The time for fracture union averaged 16 weeks.There were two case with the proximal locking screw outbreak.There were not postoperation compartment syndromes.neurological deficits,brocken nail etc complications.Conclusions The intramedullary nail is indicated for colsed and open tibia shaft fractures and using the distal interlocking vane is an effective means.
