口腔颌面部动静脉畸形手术治疗的改进及其疗效评价

Evaluation of effect of the surgical therapy for the oral and maxillofacial arteriovenous malformations

自１９８６年以来，作者对２３例口腔颌面部动静脉畸形的诊断和治疗进行了前瞻性研究。提出一套诊疗常规即：先行选择性颈外动脉造影术，辅助性动脉栓塞术，病炷根治性切除术；然后对病灶面积大、缺损较严重的创面行立即修复术。获得了令人满意的疗效。全部病例术中计算出血量；全部病人获得随访结果：最长６年１０个月、平均４年３个月，复发率８．６％（２／２３）．作者还就辅助性动脉栓塞术和传统的结扎术疗效进行了比较；结论是二者存在显著差异，前者明显优越。

Abstract Since 1986,the dignosis and treatmnt of 23 patients with oral and maxillofacial malformations have been prospectively studied.We have put forward a planned managing programme including the selective external carotid arterograph,accessory preoperatively arterial embolization,radical resection of lesion and immediate reconstruction of defect after primary resection for oral and maxillofacial arteriovenous malformations.We have got a satisfactory clinical effect.All these patients were followed up.The longest followup time was 6 years and 10 months.The average follow-up time was 4 years and 3months.The rate of recurrence was only 4.3% (1/23).In this paper,we compared the different effect between the accessory arterial embolization and the arterial ligation. The result of comparing between two methods was that the arterial embolzation was superior to the arterial ligation.
