尼莫地平治疗高血压脑出血效果观察

EFFECT OF NIMODIPINE ON HYPERTENSIVE CEREBRAL HEMORRHAGE

①目的 探讨尼莫地平对高血压脑出血血肿吸收及神经功能康复的影响。②方法 选择发病 6h以内的高血压脑出血病人 ,随机分为对照组和治疗组 ,治疗组应用尼莫地平持续泵入 ,10mg/d(5mL/h) ,连用 10d改为口服尼莫地平 ,每次 30mg ,每日 3次 ,共用 1月。对照组除不用尼莫地平外 ,其他治疗同治疗组。比较两组病人的血肿、水肿吸收情况 ,以及不同时期ESS评分、Bathel指数 (BI)。③结果 2周时 ,治疗组ESS评分高于对照组 ,差异有显著性 (t=2 .13,P <0 .0 5 ) ;血肿 +水肿体积较对照组小 ,差异有显著性 (t =2 .0 7,P <0 .0 5 )。 4周时 ,治疗组血肿体积和血肿 +水肿体积均较对照组小 ,差异有显著性 (t=2 .0 5、2 .80 ,P <0 .0 1、0 .0 5 )。治疗组 1、3月ESS评分和BI与对照组比较差异有显著性 (t=2 .0 4～ 2 .81,P <0 .0 5、0 .0 1)。④结论 早期应用尼莫地平可加速高血压脑出血病人血肿的吸收 ,进而促进神经功能的恢复。

Objective\ To observe the effect of nimodipine on the treatment of hypertensive cerebral hemorrhage (HCH). Methods\ Patients with HCH in six hours were randomly divided into treatment group and control group. Nimodipine, 10 mg/d ( 5 mL/h ), was pumped continuously into the patients in the treatment group for 10 days. Then nimodipine, 30 mg, t.i.d., was taken orally for one month. The absorption of hemorrhage and edema were observed and compared between the two groups, ESS and Bathel index (BI) were determined to evaluate the neurological deficiency and life abilities of the patients.\ Results\ ESS of the treatment group was higher than that of the control group at the second week ( t=2.13, P <0.05). The hemorrhage and edema were better absorbed in the treatment group ( t=2.07, P <0.05), especially at the fourth week( t=2.05,2.80, P <0.05, 0.01 ). The ESS and BI indicated the significant improvement of neurological function and the life abilities of the patients in the treatment group ( t=2.04-2.81, P <0.05,0.01).\ Conclusion\ Early use of nimodipine in HCH may benefit the absorption of hemorrhage and edema, and help to improve neurological functions of the patients.
