摘要
①目的 探讨胰岛素抵抗 (IR)在动脉粥样硬化性血栓性脑梗死 (ATCI)发病中的作用及其与血浆脂质含量的关系。②方法 测定ATCI病人及正常对照组空腹血糖 (FPG)、空腹血清胰岛素 (FINS)及血浆脂质成分 ,并计算胰岛素敏感指数 (ISI) ;对ATCI病人ISI与血浆脂质含量的相关性进行分析。③结果 与正常对照组比较 ,ATCI病人FINS显著升高 (t=7.2 9,P <0 .0 0 1) ,ISI显著降低 (t=6 .2 0 ,P <0 .0 0 1)。ATCI病人ISI与高密度脂蛋白胆固醇、载脂蛋白A 1呈显著正相关 (r =0 .4 2、0 .34,P <0 .0 1、0 .0 5 ) ;而与三酰甘油、低密度脂蛋白胆固醇呈显著的负相关 (r=- 0 .39、- 0 .2 8,P <0 .0 1、0 .0 5 )。④结论 ATCI病人存在IR ,且IR与多数血浆脂质成分有显著的相关性。
Objective\ To investigate the role of insulin resistance (IR) in the pathogenesis of atherothrombotic cerebral infarction (AT CI) and its relation to plasma content of lipid.\ Methods\ Fasting plasma glucose (FPG), fasting insulin (FINS) and components of lipid were determined in ATCI patients and normal controls, and in sulin sensitivity index (ISI) was calculated. The correlation between ISI and pl asma lipid level was analyzed as well.\ Results\ FINS level in the patients was significantly elevated com pared with the normal controls ( t=7.29, P <0.001), while ISI was markedly dec reased ( t=6.20, P <0.001). ISI in ATCI patients showed significantly positive correlation with HDL C and apoA 1 ( r=0.42, 0.34; P <0.01, 0.05), while neg atively correlated with TG and LDL C ( r=-0.39, -0.28; P <0.01, 0.05). \ Conclusion\ IR exists in patients with ATCI, which is highly corre lated with most components of lipid in plasma.
