缺血—再灌注时异常双极电图与心律失常

Abnormal Bipolar Electrogram and Arrhythmias During Myocardial Ischemia and Reperfusion

摘要 为了探讨缺血—再灌注心律失常机制,对20只犬建立缺血—再灌注模型,应用心电活动三维标测系统,以3水平双极针电极记录不同深度心肌的双极电图。结果表明,缺血时缺血区双极电图振幅与时间比,呈现4~6、18~20分钟两个谷值期,两期内心律失常的检出率有增高趋势。缺血及再灌注时缺血区双极电图有时记录到延迟激动及分裂电活动,这一异常电活动与心律失常密切相关。因此,多层次心肌标测技术可能是探讨缺血—再灌注心律失常机制的可靠及精确手段。 To characterize the eleetrophysiologic mechanisms underlying ischemia and reperfusion, a study was undertaken with a three-dimensional mapping system and simultaneous threelever transmural recordings from 42 bipolar sites. The results showed that bipolar electrographic amplitude and duration ratio of ischemic zone during coronary occlusion presented two valley periods which occurred 4-6 and 18-20 minuts, with a increase of arrhythmias. The delayed and fragmentized activity could be recorded from ischemic zone during ischemia and reperfusion. The abnormal electrogram was highly correlation with arrhythmias. So multiple-lever transmural recordings may be the precise and reliable way to study the mechanisms for ischemia and reperfusion.
