摘要
目的介绍一种在食管贲门癌术中置入十二指肠营养管的简便、安全、有效的方法,并探讨其在食管贲门癌术后早期营养支持中的意义。方法90例食管贲门癌患者于术中,将食管镜经胃残端通过幽门到达十二指肠球部,再通过食管镜内腔置入十二指肠营养管;于术后24h开始滴注或推注氨素营养液,每天50～100g,直至术后第10～12天完全经口进食。结果90例食管贲门癌术中置入十二指肠营养管全部成功,每例耗时仅4～8min,术后早期营养状态良好,无吻合口瘘、胃梗阻、肠梗阻等并发症。结论经食管镜置入十二指肠营养管的方法简便、安全、实用、经济,该法对于食管贲门癌患者术后施行早期肠内营养支持、节约住院费用、减少吻合口瘘等并发症具有十分重要的意义。
Objective A simple,safe,effective way of placing duodenal catheter for n ut rition support during gastrointestinal carcinoma operation was introduced,and the significance of this way for early enteral nutrition was also discussed.Met hods Ninety patients with esophageal carcinoma were placed with duodenal cathe ters through stomach stump by esophagogastroscope.50～100g Ensure nutrien t liquid was dripped24hours after operation for10～12postoperative days. Results The duodenal catheters were placed successfully in all cases.It took on ly4～8minutes for each patient.The nutritional status of each patient was ve ry good and no complications happened.Conclusions The way of placing duodenal catheter through esophagogastroscope was a simple,safe and cheap methods.It sh ould have an important role in early nutrition supporting,decreasing medical c ost and complications after esophageal carcinoma operation.
