N-乙酰半胱氨酸对低温缺血再灌注未成熟兔心肌钙转运功能影响的研究 被引量:2

An experimental study of the influence of N-acetyl-L-cysteine on calcium transport in hypothermic ischemia-reperfused isolated immature rabbit hearts

摘要 目的 对比观察附加 0 4mmol/LN 乙酰半胱氨酸 (N acetyl L cysteine ,NAC)心脏停搏液 (NAC组 )对低温缺血再灌注未成熟兔心肌钙转运功能的影响。方法 采用Langendorff Neeley离体左心做功模型 ,离体心脏缺血 12 0min ,再灌注 6 0min ;分离出缺血 再灌注未成熟左室心肌肌浆网 ,测定其总巯基 (totalsulfhydryl,TSH)、非蛋白巯基 (non proteinsulfhydryl,NPSH)及丙二醛 (malondialdehyde,MDA) ,Ca2 + ATP酶活性及其钙摄取率 (calciumuptake)。 结果 NAC组 ,再灌注后心脏功能恢复明显优于ST组 ;NAC组TSH ,NPSH含量也明显高于ST组 ,同时SR MDA含量也明显低于ST组 ;NAC组肌浆网Ca2 + ATP酶活性经历缺血 再灌注后有所下降 ,但明显高于ST组 ;NAC组与ST组肌浆网钙摄取率在缺血 再灌注后略有下降 ,但无显著差异。结论 附加NAC可显著改善St.Thomas液对低温缺血再灌注未成熟心肌的钙转运功能。其机制与NAC提高缺血再灌注未成熟心肌的还原型巯基水平 ,增强心肌抗氧化损伤能力 。 Objective To evaluate the influence of St.Thomas cardioplegia supplemented with 4mmol/L NAc(NAc group)on the calcium handling in hypothermic ischemia reperfused immature hearts compared with those of St.Thomas cardioplegia(ST group).Methods The Langendorff Neeley isolated perfused working model was applied.Cardiac sarcoplasmic reticulums were isolated from the ischemia reperfusion myocardium.Then the content of the TSH,NPSH,calcium uptake rate and the activity of Ca 2+ ATPase were measured.Results (1)Recovery of cardiac function during reperfusion was improved significantly in NAc group( P <0 01).(2)Compared with the Thomas group,the SR TSH、NPSH contents postreperfusion were maintained in a higher level in the NAc group( P <0 05),meanwhile the SR MDA content was lower in NAc group( P <0.01).(3)Postreperfusion,calcium uptake decreased slightly,there was no significance between the two groups( P >0.05);the activity of SR Ca 2+ ATPase in NAC group was higher than that in ST group during ischemia reperfusion( P <0.01).Conclusion St.Thomas cardioplegia supplemented with NAc improve myocardial calcium handling of St.Thomas solution in the ischemia reperfused immature myocardium,which might be related to maintenance the reduced thiol pool in the immature myocardium and enhancing anti oxidative capacity;attenuating inhibition of the SR Ca 2+ ATPase.
