腰腹肌肌力训练改善腰椎间盘突出症患者腰椎功能及预防复发的作用 被引量:24

摘要 目的:探讨腰腹肌肌力训练改善腰椎间盘突出症患者腰椎功能,预防复发的作用。方法:将86例经治疗并取得治愈和显效的腰椎盘间突出症患者按治疗结束的时间为序随机数字表法分为训练组和对照组,训练组进行腰腹肌肌力训练,对照组只给予腰部防护知识教育。随访1年观察两组复发率,并分别于治疗后6和12个月进行腰椎功能评定。结果:训练组1年内的复发率明显低于对照组(χ2=7.76,P<0.01)。两组患者治疗结束后即时的腰椎功能评分经t检验差异无显著性(t=0.69,P>0.05);训练组治疗后6和12个月时腰椎功能评分较治疗后即时增高,差异有显著性(t=1.92,1.95,P<0.05),6和12个月的评分差异无显著性(t=0.65,P>0.05);对照组治疗后6和12个月时的腰椎评分与治疗后即时相比降低,差异有显著性(t=2,13,1.87,P<0.05)。6和12个月的评分相比差异不明显(t=0.57,P>0.05);训练组治疗后6和12个月的腰椎功能评分分别明显高于对照组同期评分,差异有非常显著性(t=2.83,2.79,P<0.01)。结论:腰腹肌肌力训练是腰椎间盘突出症患者改善腰椎功能、预防复发的有效方法。 AIM: To investigate the utility of lumbar dorsal and abdominal muscles strength training for improving lumbar function and preventing recurrence in patients with lumbar disc herniation (LDH). METHODS:Eighty six patients with LDH that had been cured and gained the notable effect were randomly divided into training group and control group according to the time after treatment.The patients of the training group trained for strength of lumbar dorsal and abdominal muscles and control group learned the methods of lumbar protection.The recurrence ratio of LDH was observed by 1 year follow up, and lumbar function assessment was employed after 6 and 12 months respectively.RESULT:The recurrence ratio of training group was significantly lower than that of the control group within 1 year (χ 2=7.76, P< 0.01).There was no significant difference in the scores of lumbar function assessment between the two groups immediately after treatment (t=0.69,P >0.05). After 6 and 12 months the training group was higher and the control group was lower in the scores of lumbar function assessment compared with those immediately after treatment respectively(t=1.92, 1.95, P< 0.05;t=2,13,1.87,P< 0.05), but there were not significantly different between those before and after treatment in the two groups respectively(t=0.65,P >0.05;t=0.57,P >0.05).Compared to the control group, the scores of the training group were significantly higher at 6 and 12 months after treatment respectively (t=2.83, 2.79, P< 0.01).
