碱反流与食管癌

摘要 碱反流,亦称十二指肠胃食管反流(Dnodeno gastroesophageal reflux,DGER),指十二指肠内容物反流至食管,在某些生理情况下偶有发生,对机体无损害;消化道手术或某些疾病导致胃肠结构异常或动力紊乱,DGER发生频率增高,持续时间延长。与Barnett食管、食管腺癌的发生密切相关。Barrett食管是一种癌前病变。由内镜证实的Barrett食管,食管癌的发生率为0.8%/年,是正常人群的30倍-50倍。伴有肠化的Barrett食管是食管腺癌发生的重要因素。DGER是引起Barrett食管的原因之一,胃大部切除后的患者,食管癌多发生于食管下段1/3,多发生在Barrett食管的基础上,多为腺癌。
