带蒂胸大肌皮瓣在口腔颌面部缺损修复中的应用

Application of the Pectoral Major Myocutanous Flap in Oral and Maxillofacial Region Reconstruction

目的:探讨带蒂胸大肌皮瓣在口腔颌面部肿瘤切除后组织缺损一期修复的方法及治疗效果。方法:15例均采用带蒂胸大肌岛状复合组织瓣一期修复口腔颌面部组织缺损,其中舌癌5例,颊癌4例,口底癌3例,牙龈癌2例,电击伤1例。结果:15例胸大肌皮瓣全部成活,无1例发生全部或部分皮瓣坏死,患者获得满意的外观和功能。结论:带蒂胸大肌皮瓣在口腔颌面部缺损的修复中具有很大的灵活性,能修复口腔颌面部较大面积的缺损,且安全可靠,瓣的成活率高。

Objective:To discuss the methods and results of the treatment in use of the pectoral major myocutanous flap to reconstruct the large defect that results from the resection of oral and maxillofacial region cancer.Methods: Fifteen patients were undergone oral and maxillofacial region reconstruction in use of the pectoral major myocutanous flaps. Among these patients, there were five cases with tumors of the tongue, four cases with tumors of the bucca, three cases with tumors of the mouth floor, two cases with tumors of the gingiva, and one case of electric wound. Results: All the 15 pectoral major myocutanous flap survived completely, without total or partial necrosis.The relative satisfactory appearances and functions were achieved in all fifteen pateints. Conclusions: Pectoral major myocutanous flap is a versatile workhorse flap in the reconstruction of oral and maxillofacial region defects. It can reconstruct the large defect that result from the oral and maxillofacial region, and it is very safe and reliable.The survival rate of the flap is high.
