局部浸润阻滞为主治疗急性软组织损伤的临床研究

目的 探讨局部浸润阻滞为主治疗急性软组织损伤的效果。方法 急性软组织损伤病人139例,分成两组,试验组(n=76),用镇痛复合液根据部位不同用生理盐水稀释至20～30ml,以最痛点为穿刺点,行扇形浸润注射,结合于热敷、适度制动、局部按摩等理疗。5～10天后,根据疼痛缓解情况,VAS降低≥5的病人无需进一步治疗,VAS降低<5的病人进行第二次治疗。对照组(n=63):组成与实验组相似。接受NSAIDs类药物治疗,理疗措施同实验组。结果 试验组效果优良率1周80％,2周92.1％,2周有效率100％,未见明显的不良反应。对照组优良率1周30％,2周57.1％,2周有效率76.1％,15例(23.8％)出现消化系统的不良反应。结论 局部浸润阻滞治疗效果确切,治愈率高,未见明显的治疗并发症,病程缩短,病人乐于接受,是一种较好的治疗急性软组织损伤和疼痛的方法。

Objective To study the clinical effect of local infiltration block on acute tissue's traumatic pain in out department patients. Methods 139 adult patients with acute tissue's traumatic pain in outpatient were randomly divided into 2 groups according to different treatment scheme:local infiltration block group(group A)and routine therapy group(group B).Group A(n= 76):patients, with acute myofascial syndrome of back or neck, trauma of joint's ligament etc,are performed local infiltration bolck or tender point block with an analgesic compound of lidocaine.dexamethasone, triamcinolone acetonide, Vitamin B12 and Vitamine B1 . In gropu B(n = 63)medications(NSAIDs)combined with physical therapy were given to decrease inflammation. The clinical and analgesic effect were evaluated at 1 week and 2 weeks after treatment respectively. The complications of 2 groups were observed at the same time. Results The effective rate is after 1 week 80% (group A)and 30% (group B) ,2 weeks 92.1 % (group A)and 57.1 % (group B)respectively. The recovery of motor function was better .and the complication rate of gastrointestinal tract was lower in group A than in group B. Conclusions Local infiltration block or tender block combined with physical therapys on acute tissue's traumatic pain is an efficient method, which can not only relief acute pain but ensure the recovery of motor function.
