胃癌高发区14岁及以下儿童幽门螺杆菌感染及细胞毒素相关蛋白A阳性幽门螺杆菌感染的检测分析

THE ANALYSIS OF HELICOBACTER PYLORI INFECTION ON CYTOTOXIN ASSOCIATED GENE A IN FECTION IN CHILDREN UNDER 14 YEARS OLD

目的 对胃癌高发区 (西安地区 )≤ 1 4岁儿童进行幽门螺杆菌 (Hp)感染及细胞毒素相关蛋白A(CagA)阳性Hp感染的血清学检测 ,以了解各年龄段Hp的感染率及CagA阳性Hp的感染率。 方法 采用斑点金免疫渗滤法检测末梢血清中抗Hp尿素酶IgG及抗Hp -CagA -IgG ,共检测 1 4 1 5人 ,其中市区儿童 791人 ,市郊儿童 62 4人。 结果 小儿Hp感染并非逐龄连续性增高 ,但总体趋势是随年龄的增加而增高 ,≤ 4岁Hp的感染率在市郊明显高于市区 ;在Hp感染者中 ,市区及市郊小儿CagA阳性Hp感染率分别为2 9.60 %～ 55 .0 0 %及 2 3 .53 %～ 58.33 % ,≤ 4岁CagA阳性Hp感染率分别 37.50 %～ 55 .0 0 %及 2 3 .53 %～ 58.33 %。统计学分析两地间差异无显著性 (P >0 .0 5)。结论 本地区小儿CagA感染菌株可能为相同的Hp菌株感染 ,本地区消化性溃疡及胃癌高发可能与CagA阳性Hp菌株感染有密切关系。重视儿童期CagA阳性Hp感染的血清流行病学研究 ,对减少成年消化疾病发病率 ,提高民族健康素质具有深远的意义。

Objective To detect the prevalence of Helicobacter pylori(Hp) infection and cytotoxin associated gene A (CagA) infection in children under 14 years old. Methods Anti-urease antibodies of H.pylori and Hp-CagA-IgG in finger blood were examined by dot immunogold filtration assay (DIGFA) . This study involved 1415 children of which 791 came from city and 624 came from suburb. Results H.pylori in children had a tendency of raise infection rates with age. The positive rates of H.pylori infection under 14 years old in city were higher than that of suburb. The positive rates of CagA infection were 29.60 %～55.00% in city and 23.53%～58.33% in suburb ,children under 14 years old were 37.50%～55.00% in city and 23.53 %～58.33% in suburb, but they showed no significant difference (P>0.05). Conclusion There were similar Hp-CagA strains of children in this area. High prevalence of peptic ulcer and gastric carcinoma in this area had close relationship with Hp-CagA strains. It was important to study the CagA positive Hp infection in children with epidemiological methods. There were far-reaching significance to reduce the rates of digestive system diseases in adults.
