慢性硬膜下血肿治疗方法比较性研究

A comparative study on therapeutic method of chronic subdural hematoma

目的 比较微创冲洗引流术 (MIID)和钻孔冲洗引流术 (BHID)治疗慢性硬膜下血肿(CSDH)的疗效。方法 对 86例CSDH随机分成两组 ,一组采用微创冲洗引流术 (MIID) ,另一组采用钻孔冲洗引流术 (BHID)治疗。术后 4 8～ 72h复查头颅CT ,对血肿清除量、术后颅内血肿、颅内积气、蛛网膜损伤发生率、术后平均住院日和住院费用进行比较。结果 两组血肿清除量、术后颅内血肿发生率比较无明显差异 ;MIID组颅内积气、蛛网膜损伤发生率低、平均住院日短、住院费用少 (P <0 .0 1)。结论 MIID治疗CSDH较BHID创伤小。

Objective To compare curative effect of two different kinds of methods on chronic subdural hematoma (CSDH). One was minimally invasive irrigation and drainage (MIID) and the other was burr hole irrigation and drainage (BHID).Methods 86 patients with CSDH were divided into two groups randomly. 43 cases were treated by MIID, the rest by BHID. All patients were scanned with CT in 48～72 h postoperation. The two treatment methods through 6 index (volume of hematoma excavated, and the incidence of intracranial hematoma , pneumatocephalus, and arachnoid damage, the average duration of postoperative hospitalization and cost in hospital) were compared. Results No significant difference was found between the two groups in volume of hematoma excavated and the incidence of intracranial hematoma. However, short duration and less cost in hospital , low incidence of intracranial hematoma, pneumatocephalus and arachnoid damage were observed in group of MIID than BHID group ( P <0.01).Conclusion There were less damage and lower occurrence of complication in group with MIID than in group by MIID.
