兔胚胎脊髓移植与自体神经移植修复坐骨神经缺损的比较研究

摘要 目的 探讨兔胚胎脊髓 (ESC)移植修复周围神经缺损的可能性。 方法 手术造成成年家兔双侧坐骨神经缺损 ,左侧选用胎兔脊髓桥接 ,右侧为自体神经桥接 (对照组 )。对实验动物在免疫学、电生理学、肌湿重、组织学等方面进行了检测。 结果 术后血中白细胞介素 (IL) -2、IL - 6、IgA、IgG、IgM的含量分别为 (0 .14± 0 .0 1) pg/ml、(0 .2 6± 0 .0 3)pg/ml、(0 .0 6 9±0 .0 0 4 ) g/L、(3.0 5± 0 .6 4 ) g/L、(0 .2 2± 0 .11) g/L ;神经传导速度 (NCV)为 35 .7~ 4 1.6m/s ,运动神经末端潜伏期 (Lat)为 1.31~ 1.5 1m/s,运动神经电位波幅 (Amp)为 0 .5 0~ 0 .71mV。与术前或对照组比较 ,差异无显著性意义 (P >0 .0 5 )。光镜、电镜观察胚胎脊髓移植段及远侧段神经生长良好 ,神经纤维数量为 15 .32~ 19.18,与对照组比较 ,差异无显著性意义 (P >0 .0 5 )。辣根过氧化物酶 (HRP)逆行示踪 ,可在脊髓前角发现被标记的神经元。 结论 兔胚胎脊髓移植与自体神经移植修复坐骨神经在免疫学、电生理学、肌湿重、组织学及功能恢复方面无明显差异。 Objective To study the feasibility of embryonic spinal cord (ESC) transplantation in repairing peripheral nerve defects in rabbits. Methods The defects of 8 mm, 16 mm and 32 mm were created on the bilateral sciatic nerves by surgery in the rabbits. The left defects were bridged by ESC of rabbit with the gestational age of 25-28 days. The right defects were bridged with autogenous sciatic nerves and used as control group. The rabbits were observed upon aspects in immunology, electrophysiology, muscular humidity and density, and histology. Results The postoperative contents of interleukin-2 (IL-2),IL-6, immunoglobulin A (IgA), IgG and IgM were (0.14±0.01) pg/ml, (0.26± 0.03) pg/ml, (0.069±0.004) g/L, (3.05±0.64) g/L and (0.22±0.11) g/L respectively. Nerve conduction velocity (NCV), motor nerve end latency (Lat) and motor nerve potential amplitude were 35.7-41.6 m/s, 1.31-1.51 m/s and 0.50-0.71 mV, respectively. There was no significant difference compared with pre-operation and control group (P>0.05). The light and electron microscopies showed that the nerves in the transplanted embryonic spinal cord and the distal nerves grew well with nerve fibers for 15.32-19.18, with no significant difference compared with the control group (P>0.05). The stained neuronal cells were seen at spinal cord anterior horn under the reverse tracing by horse radish peroxidase (HRP). Conclusions There is no significant difference in repair of peripheral nerve defects between embryonic spinal cord transplantation and nerve allograft in the aspects of immunology, electrophysiology, muscular humidity and density, and histology.
