LASIK术后使用双氯芬酸钠与氟美瞳的比较

Comparison of topical adminstration of diclofenac and flurometholone after LASIK

目的 比较 L ASIK术后使用双氯芬酸钠与氟美瞳对抑制屈光回退的影响。方法 回顾性分析在2 0 0 2年 6月至 8月在徐州市眼病防治研究所接受 L ASIK手术的 2 1例患者 (38只眼 )。将患者分为两组 ,一组为双氯芬酸钠组 ,共 8例 (16只眼 ) ,术前平均等值球镜度 (- 6 .39± 1.19) D;另一组为氟美瞳组 ,共 13例 (2 2只眼 ) ,术前平均等值球镜度 (- 6 .47± 1.15 ) D。两组间 ,术前的屈光度无统计学差异。患者随访半年 ,比较这两组患者在术后10天、1、3、6月屈光度。所获取的数据在统计学上采用 t检验进行分析。结果 双氯芬酸钠组与氟美瞳组在术后早期 ,屈光度均呈一个低度近视或正视状态。术后 10天 ,平均屈光度分别为 (- 0 .0 2± 0 .41) D、(- 0 .15± 0 .5 4) D。随着时间推移 ,逐步向近视方向发展 ,在术后 6月 ,两组平均屈光度分别为 (- 0 .6 5± 0 .5 6 ) D和 (- 0 .47± 0 .5 9) D。无论术后 10天、1、3和 6月 ,两组间的屈光度均没有统计学显著性意义。但是 ,术后 ,双氯芬酸钠组总的屈光回退度要高于氟美瞳组。结论 屈光手术以后 ,对一些不适合使用皮质类固醇类药物的个体 ,我们可用双氯芬酸钠代替 ,以抑制屈光手术后的屈光回退。

Objective To compare the impact of topically administering Diclofenac sodium and Flurometholone eye drops of inhibitting the regressions after LASIK.Methods This is a retrospective study including 38 eyes of 21 patients who undertook LASIK in Xuzhou Eye Institute from 2002.6 to 2002.8.The patients were divided into two groups.One group used Diclofenac sodium including 16 eyes of 8 patients.Preoperatively,the mean spherical equivalent is (-6.39±1.19)D.Another group used Flurometholone including 22 eyes of 13 patients.The mean spherical equivalent is (-6.47±1.15)D.There is no significant difference of diopter between two groups before LASIK.In following up we compared the diopter between two groups after LASIK at 10 days,1 month,3 months and 6 months.Results In the early after LASIK,the diopter of two groups is low myopia or emmetropia.10 days postoperatively,the mean spherical equivalent is (-0.02±0.41)D in Diclofenac sodium group,but (-0.15±0.54)D in Flurometholone group.6 months later,the mean spherical equivalent is (-0.65±0.56)D and (-0.47±0.59)D,individually,showing a trend of myopia with the time.There were no significant difference between two groups at 10 days,1 months 3 months and 6 months.But the total regressions in Diclofenac sodium group are higher than Flurometholone group.Conclusion We can use Diclofenac sodium substitute Flurometholone to inhibit regression after refractive surgery,especially for some patients who is not suitable for using corticosteroiod.
