碘伏棉片消毒血液透析用中心静脉双腔导管的效果观察

Observation on the Effect of Disinfecting Central Venous Double-Lumen Catheter in Hemodialysis with Iodophor Cotton

目的观察碘伏棉片消毒血液透析用中心静脉双腔导管的临床效果。方法选择2016年9月1日至2016年11月30日我科颈内静脉置带涤纶套双腔中心静脉长期导管维持性血液透析的患者61例,按随机数字表法分为对照组29例和观察组32例,每例患者均观察3个月。对照组消毒时均使用医用棉签沾安尔碘消毒液消毒,观察组把医用棉签改为独立包装的碘伏棉片消毒,再比较两组患者中心静脉导管口清洁度、导管感染情况及使用两种消毒工具后护士工作满意度。结果两组在中心静脉导管口清洁度对比有显著性差异(x^2=174.495,P<0.05)。两组发生导管相关感染比较差异无统计学意义(x^2=0.05,P=0.975)。使用两种消毒工具后护士工作满意度调查,观察组87.5%高于对照组的57.5%,差异具有统计学意义(x^2=9.02,P=0.03)。结论应用碘伏棉片消毒中心静脉导管口的方法在导管口清洁度方面优于安尔碘棉签消毒中心静脉导管口的方法,且护士工作满意度高,值得临床推广应用。

Objective To observe the clinical effect of disinfecting central venous double-lumen catheter in hemodialysis with iodophor cotton. Methods A total of 61 patients with long-term catheter maintenance hemodialysis were enrolled in this study. From January 1, 2016 to November 30, 2016, 61 patients were randomly divided into control group(n=29) and control group(n=29) Observation group of 32 cases, each patient were observed for 3 months.The control group disinfected with medical disinfection swab by Anerdian disinfectant. The observation group changed the medical cotton swab to the independent packaging of iodine cotton tablets. Then, the cleanliness of the central venous catheters, the condition of catheter infection and the nursing job satisfaction after using two kinds of disinfection tools.Results There was a significant difference between the two groups in the cleanliness of the central venous catheter(x^2=174.495,P<0.05).There was no statistically significant difference between the two groups of catheter-related infections(x^2=0.05, P=0.975).After using two kinds of disinfection tools, the satisfaction degree of nurses was 87.5%,57.5% of the observation group was higher than that of the control group, and the difference was statistically significant(x^2=9.02, P=0.03).Conclusion Application of iodophor cotton disinfection central venous catheter mouth in the catheter mouth cleanliness is better than the safety of iodine swab disinfection central venous catheter approach, and nurses work satisfaction is high, which is worthy of clinical application.
