犬心肺复苏中血清儿茶酚胺浓度的变化 被引量:5

The Variation of Catecholamine Concentrations During Cardiopulmonary Resuscitation in Canines

摘要 【目的】观察心脏骤停模型犬心肺复苏过程中血清儿茶酚胺浓度的变化。【方法】通过心室内交流电刺激制造12只犬心脏骤停模型,复苏中按0.2mg/kg的肾上腺素给药,并在心肺复苏过程中按每3min抽取动脉血1次,共8次24min。应用HPLC-EC法检测血清肾上腺素、去甲肾上腺素、多巴胺的浓度。【结果】血清儿茶酚胺的浓度在心肺复苏过程中均处于高水平,其中血清肾上腺素质量浓度在心脏骤停后6min达到最高峰(5904±2619)μg/L,较骤停前上升了近1000倍;血清去甲肾上腺素质量浓度在心脏骤停后6min达到最高峰(1280±413)μg/L,较骤停前上升了近60倍。多巴胺的浓度升高较平稳,在心脏骤停后3min时处于最高峰(29±11)μg/L。【结论】在犬心脏骤停时血清儿茶酚胺明显升高,且在复苏过程中处于持续升高的水平。 Objective] To observe the variation of catecholamine concentrations during cardiopulmonary resuscitation( CPR) in canines. The canine models of cardiac arrest were established with intraventricular electric current shock, and the canines were resuscitated with administration of epinephrine (0.2 mg/kg). The artery blood samples were collected every 3 min during CPR and the catecholamine concentrations were measured with HPLC EC. The catecholamine concentrations were sustained at high levels during CPR. Peak level of epinephrine mass concentration( 5 904± 2 619) μ g/L appeared at 6 min after arrest, 1 000 times higher than before. Peak level of norepinephrine mass concentration (1 280± 413) μ g/L appeared at 6 min after arrest , 60 times higher than before. The rise of dopamine concentration was moderate, peak level appeared at 3 min after arrest( 29± 11) μ g/L. [ Conclusion] There's an obvious increase in serum catecholamine after cardiac arrest and the concentrations maintained at high levels during the whole period of resuscitation.
