左半结肠癌并急性肠梗阻一期切除吻合术的治疗体会

One-stage resection and primary anastomosis for acute obstruction of left-sided colonic carcinoma

目的 :探讨左半结肠癌并急性肠梗阻一期切除吻合术疗效和安全性。方法 :回顾性分析我院 8年来 2 8例左半结肠癌并发急性肠梗阻病人施行一期切除吻合术的临床资料。所有病例均进行术中结肠灌洗。结果 :本组无 1例发生吻合口漏及腹腔感染。 2 8例中 2例切口感染 (占 7 1%) ,1例 (占 3 6 %)因肺部感染并多器官功能衰竭而死亡。结论 :左半结肠癌并急性肠梗阻施行一期切除吻合术是安全可行的 ,但必须注意手术技术、术中结肠灌洗和严格掌握适应证。

Objective:To explore the therapeutic outcome and safety of one-stage resection and primary anastomosis for acute obstruction of left hemicolonic carcinoma. Methods:Clinical data of 28 cases of left-sided colonic carcinoma undergoing one-stage resection and primary anastomosis were analyzed respectively.Intraoperative colonic lavage were performed in all cases.Results:No anastomotic leakage or coeliac infection occurred in this group.2 cases (7.1%) developed infection of incisional wound.1 case (3.6%) died from pulmonary infection complicated with multiple organic function failure.Conclusion:One-stage resection and primary anastomosis is a acceptable for acute obstruction of left-sided colonic carcinoma.Surgical technique,intraoperative colonic lavage and adherence to proper indications are crucial for one-stage resection and primary anastomosis.
