氯沙坦和依那普利降低血压和尿酸作用的比较 被引量:6

摘要 目的:比较氯沙坦和依那普利治疗高血压的疗效,及两者对血尿酸的影响。方法:采用随机、平行对照研究,59例高血压病人分为两组:一组给予氯沙坦50mg,qd;另一组给予依那普利5mg,bid。12周后观察二种药物的降压效果和对血尿酸的影响及不良反应。结果:治疗12周后两组血压均显著下降。氯沙坦组最高血压由(167.45±10.70/100.90±10.47)降为(146.14±14.40/83.24±9.24)mmHg;24h平均收缩压由(148.93±8.91/92.31±10.43)mmHg降为(132.62±14.40/83.24±9.24)mmHg;依那普利组最高血压由(165.17±10.16/100.60±14.49)降为(147.23±14.14/89.2±7.14)mmHg;24h平均压由(149.32±9.29/91.47±12.97)mmHg降为(133.87±13.02/83.17±8.92)mmHg;氯沙坦组血尿酸治疗后由(446.48±118.55)μmol降为(379.52±74.59)μmol,依那普利组血尿酸治疗前后为(441.30±112.15)μmol和(433.77±113.96)μmol。结论:氯沙坦和依普利那降压疗效相似,但氯沙坦有降低尿酸的作用,对于血尿酸增高的高血压病人建议选用氯沙坦。 Objective:To study the anti-hypertension effects of losartan and enalapril, and compare their effects on uric acid in blood. Methods: 59 patients with essential hypertension divided 2 groups, one group with losartan 50mg per day po, other group with enalapril 5mg twice per day po, for 12 weeks, the change of blood pressure,uric acid in blood and side-effect were recorded before and after treatment. Results:Blood pressure decreased prominently in both groups after 12 weeks treatment. Highest blood pressure was declined from (167. 45 ± 10. 70)/(100. 90 ± 10.47) mmHg to (146. 14 ± 16. 70)/(88. 69 ± 9. 28)mmHg.P<0. 05; 24h average blood was declined from (148. 93 ± 8. 91)/(92. 31 ± 10. 43)mmHg to (132. 62 ± 14. 40)/(83. 2 ± 9. 24)mm-Hg in losartan group,P<0. 05; Highest blood pressure from (165. 17 ± 10. 16)/(100. 60 ± 14. 49) mmHg to (147. 23 ± 15. 14)/(89. 2 ± 7. 14)mmHg, P<0. 05; 24h average blood pressure was delined from (149. 32 ± 9. 29)/ (91.47 ± 12.97)mmHg to (133.87 ± 13.02)7(183. 17 ± 8. 92)mmHg in enalapril group, P<0. 05; anti-hypertension effects in 2 groups were no significant difference ,P>0. 05. The concentration of serum uric acid was decreased from (446. 48 ± 118. 55)μmol/L to (379. 52 ± 74. 59)μmol/L after treatment,in losartan group, P< 0. 05; The comcentration of serum uric acid was (441. 30 ± 112. 15)μmol/L and (433. 77 ± 113. 96)μmol/L before and after treatment in enalaporil group, P>0. 05. Conclusions: Anti-hypertension effects are same in losartan groug and enalapril groug; but losartan has the effect of decreasing hyperuricaemia. We suggested it is better for essential hypertension patient with hyperuricaemia using losartan to control blood pressure.
