摘要
本实验收集126例住院新生儿患儿,根据疾病程度分为:肺炎组52例,肺炎并呼衰Ⅰ型48例及肺炎并呼衰Ⅱ型26例。健康足月新生儿30例为对照组。应用森迪2600小儿肺功能仪,在潮气状态下进行肺功能测定,连续记录流速-容量环(TBFV)。结果显示,TV、PTEF、Crs、Rrs等肺功能指标组间分析差异有显著性,P<0.05。呼衰治疗前、后肺功能有所改善(P<0.01),但肺功能仍未恢复正常。结论:呼衰患儿尽管临床及血气恢复正常,而肺功能恢复正常需要一定时间。TBFV可了解呼衰患儿肺功能基本状态,明确肺功能障碍的程度及类型,指导临床合理选择最佳治疗方案,特别在机械通气时,肺功能监测尤为重要。
To study the character of pulmonary function in neonates with respiratory failure and its clinical value, 126 neonates were studied. According to the degree of pneumonia, the newborns were divided into 3 groups, i e pneumonia group, Type I respiratory failure group and type II respiratory failure group. Pulmonary function in tidal state was investigated by Sensor Medics-2600 infant pulmonary monitor in 126 neonates. Result showed tidal volume, respiratory rate, peak tidal expiratory flow, lung compliance and respiratory resistance were significantly different among groups P < 0.05. The newborns with respiratory failure had much better lung function after treatment. Therefore, although the newborns with respiratory failure were recovered in clinical sign and blood gas, but lung function can't be improved soon. Tidal breath flow volume(TBFV) can be used to evaluate the pulmonary function basic state in infant with respiratory failure, to define the degree and type of pulmonary function obstruction. It is helpful for doctor to choose rational treatment plan.
