改良式腹膜外剖宫产120例临床分析

Clinical analysis of ameliorative extraperitoneal cesarean section in 120 cases

目的:探讨改良式顶侧联合法腹膜外剖宫产术的临床疗效。方法:观察组(120例)改良式顶侧联合法腹膜外剖宫产术,对照组218例中108例行子宫下段剖宫产术,110例行新式剖宫产术,观察组采用在膀胱顶上剪开腹横筋膜,不下推膀胱,减少对膀胱的刺激及手术步骤,使手术简便化。结果:观察组与时照组在胎儿娩出时间及总手术时间、术中新生儿评分、术中出血等方面比较无显著性差异(P>0.05),术后胃肠功能恢复,术后乳汁分泌情况两组比较有非常显著性差异(P<0.01)。结论:具有手术简便、层次清楚,术中、术后并发症少,恢复快等优点。

Objective To study the clinical effect of the ameliorative top combined side extraperitoneal cesarean section. Methods Experimental group (120 cases ) receiving ameliorative top combined side extraperitoneal cesarean section were compared to 108 cases receiving traditional intraperitoneal cesarean section and 110 cases receiving neotype cesarean section (control group). The experimental group were cut into the transverse fascia above the urinary without pushing down bladder, to reduce chance to stimulate the bladder. The operative process was simplified. Results There was no difference between the experimental group and the control in the delivery time of the fetus,the total time of hte operation,operative bleeding, postoperative temperature operature and Apgar scores( P >0. 05). But there was difference in postoperative recovery of the bowel movement( P<0. 01)and postoperative milk secretion( P<0. 01). Conclusion With the advantages of simple operative process,clear anatomical exposure,less operative complications and sooner recovery,the new method is worth advocating.
