摘要
目的 为终板池相关手术提供显微解剖资料。方法 在手术显微镜下逐步观测14个尸头的终板池。结果 终板池呈高顶帐篷状,上部呈窄隙状居于隔区之间,侧壁向外下倾斜,前界为中线上的软膜融合线,后壁及后下壁为终板,底为视交叉及视神经间上膜,前下方达蝶骨平台后缘,多在视神经间形成一个隐窝,高度及长度均约2cm。主要含有前交通动脉复合体及其分支,纤维小梁一般不多。结论 多数终板池内纤维连系少,不难进入,但前交通动脉同视神经/交叉间需小心锐性分离。前交通动脉后方所见丝状物通常是至下丘脑的小穿支,多不是纤维小梁,不要轻易切断。Heub-ner回返动脉可居于A1段前方、外上方或后方,多不难分开。
Objective To supply microanatomical data for operations related to lamina terminalis cistern. Methods Lamina terminalis cistern was dissected and measured progressively in 14 cadaver's heads by using an operating microscope. Results Lamina teminalis Cistern was like a tent with high crown. Its upper part was like a gap between bilateral septic areas. The lateral walls inclined laterally and inferiorly. The anterior border was a midline along which two sides of pia mater adhered. The posterior and posterior -inferior wall was lamina terminalis. The bottom was optic chiasma and the arachnoid stretching between optic nerves. The cistern extended anterior -inferiorly to limbus sphenoidalis. It formed a recess between optic nerves in most cases. Both the mean height and length were about 2cm. Anterior cerebral-anterior communicating-artery complex and its fine branches were main contents of the cistern. The fibrous trabeculae were few in most cisterns. Conclusions Fibrous links were few in most lamina terminalis cistern, so it could not be difficult to enter the cistern. But careful and sharp dissection was important between anterior communicating artery and optic nerve/ chiasma. The threadlike things seen behind anterior communicating artery were generally rather perforating branches to hypothalamus than fibrous trabeculae, so they should not be cut rashly. Recurrent artery of Heubner courses anterior, superior or posterior to Al segment of anterior cerebral artery, so separation of these vessels was not difficult in most cases.
