脑外伤尿微量蛋白变化与GCS评分的关系

摘要 目的 评价脑外伤患者尿微量蛋白测定的临床意义。方法 根据GCS评分将急性脑外伤患者分为特重型组、重型组、中型组、轻型组 ,检测尿α1 微球蛋白 (α1 MG) ,微量白蛋白 (MA) ,转铁蛋白 (TRF)和免疫球蛋白G(IgG)。 结果 相关分析显示 ,α1 MG、MA、TRF和IgG与GCS均呈负相关。应用多元逐步回归分析法显示 ,α1 MG和IgG值与GCS计分值密切相关。结论 脑伤后早期尿α1 MG ,MA ,TRF和IgG检测可作为脑外伤程度诊断的辅助指标 ,对诊断脑外伤与肾损害的程度有一定参考价值。 Objective To evaluate the clinical significance in the detection of uromicroprotein levels in acute head injured patients.Methods The patients with head injury were divided into 4 groups according to GCS(Glasgow coma scale) score: the most severe head injury group,severe head injury group,moderate head injury group and mild head injury group.The levels of urinary α 1 microglobulin(α 1 MG), microalbumin(MA) , transferring(TRF) and immunoglobulin G(IgG) were determined in all patients. Results The correlation analysis showed that the levels of uromicroprotein were negatively correlated with GCS score.Multiple stepwise regression was used to reveal that the levels of α 1 MG and IgG were closely related to the GCS score.Conclusion The determinations of uromicroprotein are useful for evaluating the severity of brain damage in patients with acute head injury and its kidney damage.
