鼻中隔穿孔手术治疗41例报告

Surgical management of nasal septal perforation:with an analysis of 41 cases

目的 :探讨手术治疗鼻中隔穿孔的最佳径路和方法。方法 :鼻中隔穿孔 4 1例中 ,经前鼻孔径路 10例 ,经鼻翼切开径路 16例 (联合内镜 1例 ) ,蝶形切开径路 6例 ,鼻内镜下径路 8例 ,唇下径路 1例 ,手术均在局麻下进行 ,31例用复合瓣修补 ,用耳脑胶固定。结果 :经前鼻孔径路单层瓣膜修补 10例中 ,4例再穿孔 ,采用鼻翼切开 ,鼻翼鼻小柱蝶形切开 ,鼻内镜下径路 ,复合瓣修补 ,耳脑胶固定治疗 ,均一期愈合。结论 :对不同的鼻中隔穿孔要用不同的手术径路 ,复合瓣修补 。

ObJective: To investigate the most practical methods and surgical pathway of nasal septal perforation.Method:Among 41 cases with nasal septal perforation,31 cases were operated using compound flap mend and ear-brain glue fixation by different surgical pathway.Results:10 cases were operated using simplex mend from anterior nares.There were second limbic perforation with 4 cases. 31 cases were operated using compound flap and ear-brain glue fixation by alae nasi incision and morphabutterfly incision of alae nasi and nasal columella under endoscopy.All the patients healed in Ⅰ phase.Conclusion:Different surgical pathway in accord of different nasal septal perforation and nasal condition itself is performed.The compound flap mend and ear-brain glue fixation are the most practical methods in repairing of nasal septal perforation.
