摘要
目的 探讨CD32b(FcγRⅡ )在系统性红斑狼疮 (SLE)及狼疮肾炎 (LN)中的作用以及与疾病进展的关系。方法 采用免疫组化的方法对 5 8例LN患者及 19例非狼疮患者的肾活检冰冻切片组织进行了CD32b检测。结合LN的病理分型 ,对CD32b在各型表达的程度及表达差异进行了分析。采用流式细胞仪对 32例SLE患者及 2 0例正常人外周血的粒细胞、淋巴细胞、单核细胞的表达差异进行了分析。结果 肾组织CD32b主要表达于肾小球的内皮细胞、肾小管的上皮缘、胞浆、以及系膜细胞。Ⅳ型LN的肾小球及Ⅳ、Ⅴ型肾小管CD32b的表达较对照组明显减少 (P <0 .0 5 )。SLE患者 3种外周血细胞的CD32b的表达明显少于正常人。结论 推测CD32b在SLE的发生及LN的进展中起重要作用。
Objective To investigate the role of CD32b(FcγRⅡ) in the pathog enesis and progress of systemic lupus erythematosus(SLE) and lupus nephritis(LN) .. Methods Renal expression of CD32b was examined in 58 LN patients bi opsy specimens and 19 controls by immunohistochemical method. According to patho l ogical classes of LN,the different CD32b expression levels of five classes of LN were analyzed. Flow cytometry were used to analyze the expression of CD32b on p eripheral blood cells(including granulocyte,lymphcyte and monocyte) from 32 SLE patients and 20 healthy controls. Results CD32b expressed mainly in endothelium of glomeruli,epitheli um and cytoplasma of tubular tissues and mesangial cells. CD32b expression in gl omeruli of class Ⅳ LN and in tubular tissues of class Ⅳ,Ⅴ LN were markedly down-regulated (P<0.05). CD32b expression levels of peripheral blood granu locyt e,lmphocyte and monocyte of SLE were significantly lower than normal controls( P<0.05). Conclusion The down-regulated expression of CD32b may play an impo rtant role in the pathogenesis of SLE and the progress of LN.
