摘要
目的:探讨动态脑电图在儿童植物神经性癫痫诊断、鉴别诊断及减少误诊误治中的作用。方法:通过对246例临床诊断为植物神经性癫痫的病人进行REEG及AEEG描记,并进行对比分析。结果:本文AEEG正常162例(65.9%),异常84例(34.1%)。其中非特异性异常38例(15.4%),痫样放电46例(18.7%)。REEG院内检查111例,正常89例(80.2%),异常22例(19.8%)其中非特异性异常12例(10.8%),痫样放电10例(9.0%)。结论:AEEG可提高植物神经性癫痫的诊断阳性率及痫样波发放率。AEEG和REEG两组相比分别为X^2=7.52,P<0.01;X^2=11.46,P<0.001。均有显著性差异。同时显示AEEG可明显减少儿童植物神经性癫痫的误诊率。EEG是植物神经性癫痫诊断的重要标志之一,但要防止诊断扩大化造成的误诊误治。
Objective:To study diagnostic effect of ambulatory electroencephagram( AEEG) in children's autonomic epilepsy(EP) . Methods: Routine EEG(REEG) and AEEG were analyed and compard in 246 cases of childrens autonomic EP. Results: In AEEG of 246cases children s autonomic EP, 162(65 .9% ) cases were normal and 84(34.1 %)cases were abnormal ,38(15.4% )cases were nonspecific abnormal,46(18.7% )cases were sharp wave. In REEG of 111 cases, 89 ( 80.2 % ) cases were normal, 22 ( 19.8 % ) cases were -abnormal ,12(0.8%) cases were nonspecific 10(9.0% )cases were sharp wave. Conclusion:The study showed that AEEG can increase the diagnostic positive rate of children s autonmmic EP, and there are significant difference in AEEG and REEG (x2 = 7.52,p < 0.01 ;x2 = 11.46, p < 0.001) , AEEG can significantly decrease misdiagnostic rate in children automic EP so avoid diagnostic extending.
