改良横裁包皮岛状皮瓣尿道成形术治疗中度小儿尿道下裂

Modified Duckett for treatment of pediatric hypospadias

目的 探讨应用改良横裁包皮岛状皮瓣尿道成形术 (Duckett)治疗有阴茎下弯的尿道下裂的手术疗效。方法 71例Ⅱ型中度尿道下裂病人随机分为治疗组 35例与对照组 36例 ,分别采取改良Duckett手术和Duckett手术。结果 对照组手术成功率 5 2 8% ,并发尿瘘 13例 (36 1% ) ,尿道狭窄 3例 (8 3% ) ,尿道扩张 1例 (2 8% ) ;治疗组成功率 77 1% ,并发尿瘘 7例 (2 0 % ) ,尿道狭窄 1例 (2 9% ) ,两组相比差异有显著意义 (P >0 0 5 )。结论 通过对Duckett术式的关键环节进行改良显著地提高了手术成功率。

Objective To study the clinical effect of medified Duckett for the treatment of hypospadias.Methods Seventy-one patients with type Ⅱ moderate hypospadias were randomly divided into study group(35 cases) and control group(36 cases) and treated with modified Duckett and Duckett respectively.Results The surgical curative rate in control group was 52.8%.The complications included urine fistula(13 cases,36.1%),urine stricture(3 cases,8.3%),urine expand(1 case,2.8%).The surgical curative rate in study group was 77.1% with complications of urine fistula(7 cases,20%),urine stricture(1 case,2.9%).There was significant difference between study and control group(P<0.05).Conclusion Modified Duckett operation can singnificantly improve the surgical curative rate.
