促进组织工程化骨血管化的研究进展与策略 被引量:1

摘要 血管化和骨再生是骨愈合过程中的两个最基本环节,体外构建的组织工程化骨,植入体内后同样必须迅速建立充分的血供,为种子细胞功能活动提供充足的营养。目前随着采用组织工程学技术修复骨缺损研究的不断深入,出现了多种促进组织工程化骨血管生成的措施,包括:①包裹血管束法。②预构带血管蒂的组织工程化骨肌骨瓣。③带血管蒂筋膜包裹法。④复合血管内皮细胞。⑤应用促血管生成的生长因子。但这些方法均存在一定程度的缺陷。血管生成是一个包括细胞、生长因子、细胞外基质、生物力学刺激等多因素相互作用的复杂过程,可以概括为血管新生启动阶段、进展阶段、形成阶段、塑形和改建阶段。从血管生成的机制出发,以临床应用为目的,新的研究措施将向两个方面深入发展:①体外构建组织工程化骨时同时构建人工网状脉管系统。②更合理地利用促血管生成的生长因子或细胞。这些技术方法的完善,将为扩大组织工程化骨的应用范围和提高修复的效率,创造有利条件。
