摘要
目的 探讨血小板源性内皮细胞生长因子 (PD ECGF)与膀胱移行细胞癌 (BTCC)的相关性。 方法 采用免疫组化方法对BTCC组织中PD ECGF、血管内皮生长因子 (VEGF)表达及微血管密度 (MVD)进行观察 ,结合 5 2例患者 5年随访结果分析。 结果 5 2例患者中有完整随访资料者 5 0例 ,术后 5年无瘤生存率 4 2 .9% (2 1/ 4 9) ,总生存率 78.0 % (39/ 5 0 )。PD ECGF在G1肿瘤中阳性表达率 17.6 % (3/ 17) ,G2 5 9.3% (16 / 2 7) ,G3 87.5 % (7/ 8) ,P <0 .0 1。PD ECGF阳性组无瘤生存期 (31.87± 2 3.75 )个月 ,阴性组 (4 2 .19± 2 3.84 )个月 ,P <0 .0 5 ;总生存期阳性组 (4 8.87± 19.88)个月 ,阴性组 (5 7.19± 8.97)个月 ,P <0 .0 5。MVD极高组总生存期 (4 3.5 0± 2 2 .74 )个月 ,远低于高、中、低 3组的平均 (5 6 .4 6± 10 .98)个月 ,P <0 .0 5。随肿瘤MVD增高 ,VEGF阳性表达率显著增高 ,PD ECGF表达无明显增高。 结论 PD ECGF及MVD与BTCC恶性程度有显著相关性。
Objective To investigate the correlation of clinical and pathological characteristics of bladder transitional cell carcinoma(BTCC) with platelet-derived endothelial cell growth factor (PD-ECGF). Methods Expression of PD-ECGF,VEGF and MVD were examined in the specimens of 52 cases of BTCC by Envision immunohistochemical staining.All the 52 cases were followed up via interview for a period of 5 years. Results Of the 52 cases 50 provided complete relevant data.The 5-year postoperative tumor-free survival rate was 42.9%(21/49) and overall survival rate was 78.0%(39/50).The positive expression rate of PD-ECGF in G 1 tumors was 17.6%(3/17);in G 2,59.3%(16/27) and in G 3,87.5%(7/8)(P< 0.01).Tumor-free survival period was (31.87±23.75)months in PD-ECGF positive expression cases,and (42.19±23.84)months in PD-ECGF negative ones (P< 0.05).Overall survival period was (48.87± 19.88)months in PD-ECGF positive expression cases, and (57.19±8.97)months in PD-ECGF negative ones (P< 0.05). The cases with extremely high MVD had significantly shorter overall survival of (43.50± 22.74)months than the mean of others (high,middle,low MVD) of (56.46±10.98)months,(P<0.05).With increase in MVD,the positive expression of VEGF went up obviously,but that of PD-ECGF did not. Conclusions Expression of PD-ECGF and MVD may have significant correlation with biological behavior of BTCC.
