摘要
目的 :探讨胃镜直视下刷取胃粘膜脱落细胞中端粒酶活性对胃癌的诊断价值。方法 :采用TRAP -PCR -ELISA法对 5 6例胃镜下活检组织和胃粘膜刷落细胞进行端粒酶活性分析。结果 :胃癌组端粒酶活性水平 (平均A值0 .745 )明显高于胃炎组 (平均A值 0 .0 6 8)。胃刷落细胞的与活检组织的端粒酶活性水平是相近的。端粒酶在胃癌组的阳性率为 90 % ,明显高于胃刷落细胞涂片的阳性率 (6 6 .7% )与血CEA的阳性率 (5 6 .7% )。同时 75 %的胃粘膜不典型增生者其端粒酶活性水平也显著增高。结论
Objective To investigate the diagnostic significance of telomerase activity in the exfoliated stomach cells sampling from endoscopic brushing.Methods The technique of TRAP-PCR-ELISA was employed to detect telomerase activity in 56 cases with gastric lesions.Results Telomerase activity in the stomach carcinoma group(mean A 0.745)was significantly higher than that with the gastritis group(mean A 0.068).The telomerase activity of stomach exfoliated cells was similar to that of the gastric tissue. The positive rate of telomerase activity detected in the carcinoma group (90%) was significantly higher than that of cytological examination (66.7%) and blood CEA(56.7%). 75% of the cases with dysplasia was found to have positive telomarse activity.Conclusion Measurement of telomerase activity in the exfoliated cells obtained from endoscopic brushing may act as an effective and sensitive way for the diagnosis of stomach carcinoma.
