67例忽略性横位产的处理

MANAGEMENT OF PATIENTS WITH NEGLECTED TRANSVERSE LIE

1985年4月至1987年3月共收治忽略性横位产67例。其中30例在全麻下施行内倒转+臀牵引术。该术具有操作简便,手术需时短,术后产妇康复快等优点。子宫破裂16例中,施行子宫修补术10例(62.5％),施行子宫切除术6例(37.5％)。67例中经腹手术共29例,均使用Rifocine 400mg注入手术区域,并选用广谱抗生素静滴或肌注。经治疗后效果良好,患者腹部伤口均Ⅱ/甲愈合,亦无生殖道严重感染发生。

From April 1985 through March 1987, 67 patients with neglected transverse lie were admitted and treated. Among them internal version and breech traction under general anesthesia were performed in 30, which were favored due to simple management and smooth convalescence. Sixteen patients had uterine rupture, suture repair was carried out in 10 (62.5%) and hysterectomy in 6 (37.5%). Twenty-nine of 67 patients had abdominal operation with rifocine 400 mg used routinely in operative field, and intravenous or intramuscular injection of broad spectrum antibiotics simultaneously. No serious infection of genital tract was observed.
