摘要
本文介绍24例急性颅内血肿的非手术治疗体会。认为若病例选择恰当,在血肿厚度>0.8cm,中线结构移位>0.5cm的患者中,如临床症状轻且无进行性恶化,也能经非手术治疗获得成功。有4例血肿一度增大,但症状无明显恶化,治疗后于19～21天内血肿基本吸收好转出院。合并血小板减少或脑梗塞的患者预后较差。
Twenty-four patients with acute intracranial hematoma caused by brain injury were treated conservatively with intravenous mannitol and dexamethasone, coagulants and hyperalimentation. Among them 4 had hematoma thickness greater than 0.8cm and 13 had midline shift greater than 0.5cm. Patients were monitored by CT. As long as no signs of cerebral compression appeared, conservative treatment continued, even in 4 patients having temporary enfargement of hematoma between 7-12 days. Two patients died of accompanying thrombocytopenic purpura and cerebral infarction respectively.
