常规分割与后程三维适形放射治疗鼻咽癌的对比观察

Comparison of Late-course three Dimensional Conformal Radiation Therapy and Conventional Fractionation for Nasopharyngeal Carcinoma

目的 探讨后程三维适形放射治疗鼻咽癌的疗效及远期并发症。方法 68例初治鼻咽癌患者分为后程三维适形放疗 (3DCRT)组和常规分割治疗适形组 ,其中后程适形组 3 4例 ,先常规分割照射 60Gy后再 3DCRT 10～ 15Gy ,常规分割组 3 4例 ,总剂量为 70Gy。 结果 后程适形组和常规分割组的鼻咽部肿瘤完全缓解率 (CR)分别为 97.0 6%、94.12 % ;2组 1、3年肿瘤局部控制率及患者 1、3年生存率相比均无显著性差异 ,2级远期并发症也无显著差异 (P >0 .0 5 )。结论 鼻咽癌后程

Objective To evaluate the clinical efficiency and complications of late-course three-dimensional conformal radiation therapy(3DCRT) for nasopharyngeal carcinoma(NPC) patients.Methods 68 NPC patients were divided into group treated with late-course 3DCRT and group treated with conventional fractionation(CF).34 patients in late-course 3DCRT group were treated with conventional fractionation in a dose of 60 Gy first,and then followed by late-course 3DCRT protocol(10～15 Gy).Another 34 patients in CF group received a total dosage of 70 Gy.Results The complete response rates for the nasopharyngeal carcinoma after 3 months of radiotherapy were 97.06% and 94.12% for late-course 3DCRT group and CF group respectively.There was no significant difference in local control rate and one-year and three-year survival rate between 3DCRT and CF group.Late complications showed no difference between two groups(P>0.05).Conclusion Treatment with late-course 3DCRT has no significant advantage for local control rate,survival rate and late complication of NPC.
