双胎妊娠围产儿105例预后分析

Analysis of perinatal prognosis of 105 cases twin pregnancy.

【目的】 探讨 10 5例双胎妊娠孕产期并发症、分娩方式与围产儿死亡率之间的关系。 【方法】 对10 5例双胎妊娠孕产期并发症与围产儿死亡作回顾性临床分析。 【结果】 2 10例围产儿中低体重儿 96例 ,其发生率为 45 .71% ,病死率 2 18.75‰ ,明显高于体重正常新生儿病死率 ( 4 8.0 8‰ ) (P <0 .0 1) ;早产者 49例 ,早产率为 46.67% ,早产儿病死致围产儿死亡占总围产儿病死率 ( 12 3 .81‰ )的 46.15 % ;剖宫产 5 1例 ,剖宫产率为 5 0 .5 0 % ,剖宫产新生儿死亡率为 2 % ,阴道产新生儿死亡率为 11% ,两者比较差异有显著性 (P <0 .0 5 ) ;Logistic回归分析表明 :低体重、早产、妊娠并发症为围产儿死亡的主要危险因素。 【结论】 双胎妊娠应加强产前检查 ,避免胎膜早破、早产 ,及时防治妊高征等并发症 ,适当放宽剖宫产指征。

To study the relationship of the complications in twin pregnancy women with the delivery form and perinate mortality. Twins pregnant patients(105 cases) with complications and their perinates death were analyzed with retrospectively analysis. There were 96 low birth weight neonates(45.71%) in 210 cases, the mortality of these neonates were 218.75‰, significantly higher than normal neonates (48.08‰). The premature delivery rate was 46.67%, neonate mortality because of premature delivery was 46.15% in total perinate mortality. Cesarean section rate was 50.50%, the neonate mortality of vaginal delivery was significantly higher than cesarean section ( P <0.05). [Conclusion] Premature delivery and pregnancy induced hypertension syndrome are the predominant causes of twin pregnancy perinates death. We should enlarge the indication of cesarean section.
