摘要
目的 超选择双侧子宫动脉栓塞术治疗子宫肌瘤 ,测定栓塞后肌瘤缩小情况、血流动力学改变、性激素水平、肌瘤组织病理改变以评价其临床疗效。方法 采用Seldinger’s插管方法对 2 7例子宫肌瘤行子宫动脉栓塞 (u terinearteryembolization ,UAE)并行彩超检测肌瘤缩小情况、子宫血流动力学改变、性激素含量水平及病检结果分析。结果 介入栓塞治疗成功率 96 .3%(2 6 / 2 7) ;随访 3～ 17月 (平均 10 .6月 ) ,术后 3月B超示肌瘤缩小 5 0 %以上占 30 .77%(8/ 2 6 ) ,术后 6月B超示肌瘤缩小 5 0 %以上占 44 %(11/ 2 5 ) ,术后 12月以上B超示肌瘤缩小 5 0 %以上占 6 0 %(12 / 2 0 ) ,术后 12月内肌瘤完全消失 4例 ;栓塞后 2 4h、三月检测血流动力学改变 ,见动脉血流图象消失 ;性激素含量介入前后比较差异无显著性意义 (P >0 .0 5 ) ;介入后肌瘤组织病检为透明样坏死改变。结论 UAE治疗子宫肌瘤创伤小 ,痛苦少 ,并发症少 ,住院时间短 ,恢复快 ,是一种安全、有效的治疗手段。
Objective To evaluate the clinical effect of uterine artery embolization as a treatment of uterine myoma.Methods Twenty-seven patients with uterine myoma were treated by intra-arterial embolization of PVA particles.Results All cases were observed with a follow up of 3 to 17 months after the treatment.The success rate of the operation was 96.3%.The marked diminishment of tumor size(>50%)accounted in 30.77% of the patients at 3 months after the procedure, with 4 UM disappeared completely.The change of blood dynamics showed that the image of artery disappeared at both 24 hours and 3 months after operation.There was no notable difference in the level of sex hormone after UAE.Post-UAE leiomyomas showed hyaline-type necrosis.Conclusion Uterine artery embolization is a safe and effective procedure with less trauma,minimal pain,less complications and a rapid rehabilitation.
