抑肽酶和白细胞介素-1β抗体预防术后腹腔粘连的实验研究

摘要 目的:研究抑肽酶和白细胞介素-1β(IL-1β)抗体预防小鼠术后腹腔粘连的能力。方法:以BALB/c小鼠为动物模型,140只BALB/c小鼠随机分成四组:生理盐水对照组,抑肽酶治疗组,IL-1β抗体治疗组,抑肽酶联合IL-1β抗体治疗组,经过同一标准的腹膜损伤手术后,分别在关腹前腹腔内灌注生理盐水及抑肽酶和相应抗体。术后20d将小鼠处死,观察腹腔内粘连的情况,通过对粘连的程度、位置以及粘连类型的综合评价得出粘连分数。结果:抑肽酶联合IL-1β抗体治疗组的平均粘连分数和2—3级粘连发生率明显低于对照组(P<0.01),无一例小鼠出现腹壁切口疝。结论:抑肽酶和IL-1β抗体能够预防术后腹腔粘连形成,有利于病人的早日康复。 Objective:To assess ability of aprotinin and interleukin-1β (IL-1β)antibody to prevent postoperative abdominal adhesion in mice.Method:140 BALB/c mice were randomly divided into four groups: normal saline group(n=35),aprotinin group(n=35),anti-IL-1β group(n=35),and aprotinin combined with IL-1β group(n=35). After an identical peritoneal injury operation,mice were treated intraperitoneally with either normal saline or aprotinin and corresponding antibody at the time of abdominal closure.The mice were killed 20 days after operation,the abdominal incisions and the development of intraabdominal adhesions were observed. Adhesion scoring was based on an overall assessment of the extent,location,and adhesions type.Result:Aprotinin combined with anti-IL-1β group had significantly lower adhesion scores and percentage of 2-3 grade adhesions than control groups(P<0.01).No incisional hernia was found in abdominal wall in all cases.Conclusion: Intraperitoneal administration of aprotinin and IL-1β antibody can inhibit postoperative adhesion formation.Author′s address Dept. of Hepatobiliary Surgery,Xijing Hospital,Fourth Military Medical University,Xi′an,710032
