成人型白喉破伤风联合疫苗接种7～8岁儿童的临床反应和血清学效果观察

Observation of Clinical Reactions and Serological Effect of Adult Adsorbent Diphtheria-tetanus Vaccine in Children of 7-8 Years Old

采用成人型吸附白喉破伤风联合疫苗 (Td)接种 7～ 8岁儿童 ,以观察临床反应和血清学效果。结果显示 :一般反应发生率为 5 7 14 % (48/ 84 ) ,无高热 (≥ 38 6℃ )和异常反应发生 ;白喉、破伤风抗毒素免疫成功率分别为94 0 5 %和 96 4 3% ,白喉抗毒素阳性率从 88 10 %上升至 98 81% ,几何平均滴度 (GMT)从 0 12 4 0IU/ml上升为4 6 0 2 4IU/ml;破伤风抗毒素阳性率从 6 9 0 5 %上升至 98 81% ,GMT由 0 0 332IU/ml上升为 4 380 1IU/ml。结果表明 ,成人型吸附Td接种 7～ 8岁儿童反应轻微 ,血清学效果良好 ,可以代替儿童型吸附白喉破伤风联合疫苗(TD)用于 7～

We had revaccinated 84 children of 7～8 years old with adult adsorbent diphtheria-tetanus vaccine (Td), and observed their clinical reactions such as fever, swollen of the injected site, etc. and also the serological effect by indirect hemagglutination assay (IHA). The results showed that the general reaction rate of Td was 57 14% (48/84), two of the children (2 38%) had moderate and severe swollen (angio-edema), none of them had severe fever reaction or abnormal reaction. The positive seroconversion rates or the ≥4 fold rise of the anti-diphtheria toxin and the anti-tetanus toxin were 94 05% and 96 43% respectively. The positive rate of the anti-diphtheria toxin rose from 88 10% to 98 81%, the GMT from 0 124?0IU/ml to 4 602?4IU/ml. The positive rate of the anti-tetanus toxin rose from 69 05% to 98 81%, the GMT from 0 033?2IU/ml to 4 3801IU/ml. It indicated that the side effect of Td was mild and the serological effect was satisfactory, we can replace TD with Td for revaccination of the 7～8 year old children.
