21例肺部炎性假瘤的诊断与外科治疗

MANAGEMENT OF PULMONARY INFLAMMATORY PSEUDOTUMORS (REPORT OF 21CASES)

目的 :探讨肺部炎性假瘤的临床特点、诊断及外科治疗 ;方法 :对 2 1例肺部炎性假瘤临床资料进行分析 ,全组均行手术治疗 ;结果 :2 1例术后无并发症和死亡。术前仅4例 ( 1 9.0 % )诊断正确 ,余 1 7例中 7例疑为肺癌 ,1 0例误诊为结核瘤或良性肿瘤 ;结论 :该病病程长 ,症状较轻 ,从临床症状和影像学上很难与肺癌鉴别。强调术前纤维支气管镜和经皮肺穿刺检查十分重要。肺部炎性假瘤虽属良性 ,亦应尽早手术治疗。术式以局部切除和肺叶切除为主 ,尽量多保留正常肺组织。

Objective:To review the experience of surgical management of pulmonary inflammatory pseudotumors. Method:21 patients with pulmonary inflammatory pseudotumor were treated surgically from 1980 to 2002. The clinical data were reviewed. Results: The postoperative morbidity any mortality was not found in all cases. 7 cases were misdiagnosed as lung cancer before surgery and ten patients were suspected having tuberculoma and other benign lung tumor. Correct diagnosis was only made in 4 patients (19.0%). Conclusion: Preoperative diagnosis of pulmonary pseudotumor is quite difficult. Surgery should be performed as soon as diagnosis be made.
