负性心理因素与晚期癌症患者预后的关系

Effect of negative mental factors on patients with late cancer

目的:探讨晚期癌症患者的负性心理极其影响因素,为实施提升其生活质量的心理干预提供依据。方法:根据自愿受试原则,收集癌症均为Ⅲ～Ⅳ期入院治疗的晚期癌症患者200例。采用心理状况影响因素调查表及症状自评量表(symptomchecklist-90,SCL-90)调查评估癌症患者心理健康状况。结果:癌症患者SCL-90总分(151.12±24.66)明显高于常模组(131.87±28.14),差异具有显著性意义(t=2.98,P<0.01)。人际关系敏感及偏执两个因子分明显低于常模组(t=2.29,2.84,P<0.05),差异具有显著性意义。躯体化、焦虑、精神病性及抑郁4个因子分显著高于常模组,差异具有显著性意义(t=2.86,2.87,2.38,2.74,P<0.05)。负性心理影响因素以自信心、医疗费用的性质、综合治疗反应及对病情知道否差异最具显著性意义(T=-3.09,-2.91,2.69,2.29,P<0.05)。结论:晚期癌症患者普遍存在着负性心理,影响其心理健康及生活质量的提高。

AIM:To explore the negative mentality and its affecting factors,and to provide basis of mental intervention for elevating of their life quality. METHODS:200 late cancer patients of stage Ⅲ-Ⅳ were invovled volunteerly.Ef fect factors of mental state investigation scale and symptom checklist-90(SCL- 90) were adopted to evaluate the mental health state of cancer patients. RESULTS: SCL-90 total score of cancer patients(151.12±24.66) were signifi cantly higher than that of normal group(131.87±28.14)(t=2.98,P< 0.01),score of sensative and partial factors of interpersonal relation were signicantly lowe r than those in normal group(t=2.29,2.84,P< 0.05), score of somatization,anxie ty,state of mental disease,depression were significantly higher than those in no rmal group(t=2.86,2.87,2.38,2.74,P< 0.05).Confidence, kind of medical cost,com prehensive therapeutic effect and understanding degree of illness were the most significant factors of negtive psychology(T=-3.09,-2.91, 2.69,2.29,P< 0.05).
