Rest-redistribution of thallium-201 SPECT imaging in patients with coronary heart disease

摘要 目的 研究2 0 1 Tl(铊 )心肌灌注静息 -再分布显像在冠心病诊断中的应用价值。方法  89例患者 ,冠心病 73例 (分别为心肌梗死恢复期 2 4例、心绞痛 19例、胸闷 3 0例 )、其他 16例 (分别为原因不明的胸闷胸痛 10例、心肌炎3例及心肌病 3例 )。利用静息 -再分布2 0 1 TlSPECT显像 ,分别于注射后 10min和 180min进行采集。结果  89例受检者共 80 1个心肌节段 ,在早期相 ( 10min)发现 1~ 3级放射性分布减低区 42 2个节段 ,而在延时相 ( 180min)发现 1~ 3级共 3 19个节段。其中 ,5 99个节段的放射性分布减低区在延时像时无明显变化 ,164个节段的放射性分布在延时相得到部分改善或填充 (再分布 ) ,3 8个节段的放射性分布反较早期静息像减低 (新出现放射性分布减低区或评分等级 )增加(反向再分布 )。结论 2 0 1 Tl静息 -再分布显像不仅可用于评价心肌梗死后的存活心肌 ,还可用于心肌缺血的评价。 Objective To investigae the clinical valueof Rest-Redistribution of Thallium-201 SPECT in Coronary Artery Disease (CAD).Methods This study consisted of 89 patients, 73 cases with Coronary Artery Disease (24 cases with acute myocardial infarction; 19cases with angina pectoris; 30 cases with bored), 16 cases with other diseases. Rest-redistribution of Thallium-201 SPECT were performel in oupetients. Results In 89 cases, there are801 segments in all. There are 422 segments to varying degree (1-3 scores) hypoperfusion discovered in early image (10 min). Among them, 319 segments were notchanged, 164 segments improved & filled in late image (180 min), 38 segments worsen and more hypoperfusion (Reverse redistribution. RR). Conclusion Rest-redistribution of Thallium-201 SPECT is useful for distinguishingmyocardial hypoperfusion from infarction. Reverse redistribution is useful forthe degree of myocardial hypoperfusion.
