现代白内障囊外摘除及人工晶体植入术

MODERN EXTRACAPSULAR CATARACT EXTRACTION AND INTRAOCULAR LENS IMPLANTATION

报告了1985～1987年间所做的100例117眼白内障囊外摘除及人工晶体植入术,病人中老年性白内障106眼,并发性白内障6眼,外伤性白内障5眼。年龄29～80岁,平均62岁。89眼为后房型、28眼为前房型人工晶体。114眼一期植入,3眼二期植入。平均随访4.7个月,随访的101眼中89眼最佳矫正视力在0.5以上,除外7眼原有眼疾影响视力外,95％的眼获得0.5以上的视力。

117 extracapsular cataract extractions with intraocular lens implantation(ECCE-IOL) performed on 100 patients during the past 2 years were reported.Of these cataract eyes,106 were senile cataracts,6 were complicated,5 were traumatic.The age of patients ranged from 29 to 80 years,with an average of 62 years.Posterior chamber IOLs were implanted in 89 eyes,and anterior chamber IOLs were implanted in 28 eyes.114 eyes were primary implantations and 3 eyes were secondary implantations.The average time between surgery and last postoperative visit was 4.7 months.We achieved 20/40 or better final postoperative visual acuity in 89 of 101 eyes.Excluding 7 cases with pre-existing ophthalmic diseases (3 optic atrophies,2 corneal maculas,2 maculopathies),95% eyes obtained 20/40 or better final visual acuity.Pre-operative preparations and main operative steps were presented.It is emphasized that adjusting surgical microscope and using microsurgical instruments skillfully,are basical requirements for successful surgery.The lowering of IOP,complete anesthesia,total aspiration of the lens cortex,and intact posterior capsule remained,and the proper insertion of IOL,are the essential steps in the operation of IOL implantation.
