摘要 目的 研究一种治疗髌骨骨折的新的内固定方法。 方法 根据国人髌骨的统计数据 ,采用镍钛形状记忆合金制做蟹爪式聚髌器 ,并进行生物力学研究。 结果 对髌骨横行骨折 ,蟹爪式聚髌器的固定作用明显优于改良张力带钢丝固定 (P <0 .0 0 1) ;对髌骨粉碎性骨折 ,聚髌器的固定作用亦明显优于环形钢丝固定 (P <0 .0 0 1)。 结论 蟹爪式聚髌器设计符合髌骨的解剖和生物力学特点 ,其对髌骨骨折的固定强度可满足临床应用之需要。 Objective To study a new internal fixation method for the treatment of patellar fracture. Method According to the statistics of patella in the Chinese, a special apparatus NiTi shape memory alloy crabby patellar concentrator (NT CPC) was designed and manufactured for the treatment of patellar fractures. Biomechanical tests were carried out. Results The results demonstrated that the design of NT CPC could be used for the fixation of various types of patellar fractures. NT CPC revealed the special features in the fixation of comminuted fracture of patella, it can actively exert continuous and centripetal stress on the fracture lines from 8 directions. The biomechanical test revealed that the fixing efficacy with NT CPC was better than that with modified tension band wires to transverse fractures. Conclusion NT CPC is ideal for the treatment of patellar fractures, especially the comminuted ones.
