广视野系统在复杂视网膜脱离手术中的应用

Wide field inversion lens system in operation of complicated retinal detachment

目的 :探讨广视野镜倒像装置系统在玻璃体视网膜显微手术治疗复杂视网膜脱离中应用的价值。方法 :将广视野镜倒像装置系统应用于 4 6例复杂视网膜脱离手术 ,2 7例在气态下行眼内半导体激光光凝 ,其中 5例在气体联合重水下完成光凝 ;1 9例在气态下直视观察行巩膜外冷疑 ,术毕 31例玻璃体腔填充 1 6 % C3F8,1 5例填充硅油。结果 :全部病例顺利完成玻璃体切除及气态下眼内操作 ,随访 6个月至 1 5个月 (平均 1 0 .3个月 ) ,治愈率 89.1 % (4 1 / 4 6 ) ,盲目率从术前 84 .8% (39/ 4 6 )下降到术后 6月 32 .6 % (1 5 / 4 6 ) (P<0 .0 1 )。结论 :广视野镜倒像装置系统为现代玻璃体视网膜显微手术提供了一个广角视野 ,它的应用使许多高精度操作能够顺利完成 ,提高了治愈率 ,减少了盲目率 。

Objective:To discuss the benefit of wide field inversion lens system in operation of complicated retinal detachment Methods:46 eyes with complicated retinal detachment received operation,wide field inversion lens system was used.27 eyes receive intraocular photocoagulation,which was performed in air in 22 eyes, in air and heavy water in 5 eyes;19 eyes receive cryotherapy,16% C 3F 8 was injected into 31 eyes,when silicon oil was injected into 15 eyes Results: All overation succeeded at the first time.Follow up was from 6 to 15 months(10 3 month in average),rehabilitation rate is 89 1%(41/46),blindness rate dropped from 84 8%(39/46)to 32 6% (15/46)( P ≤0 01) Conclusion:Wide field inversion lens system provides a wider operational field,oculist can perform operation easier than before,it increase the rehabilitation rate,and decrease the blindness rate.It is a useful device for complicated retinal operation
