小腿后侧逆行岛状筋膜皮瓣的临床应用

CLINICAL APPLICATION OF DISTALLY BASED ISLAND FASCIAL FLAP ON THE POSTERIOR CALF

目的 :探讨小腿后侧逆行岛状筋膜皮瓣的临床应用问题。方法 :应用小腿后侧逆行岛状筋膜皮瓣修复胫前下 1/3、内外踝、踝前、跟腱及跟底创面 18例 ,皮瓣切取面积 4cm× 3.5 cm— 12 cm× 9cm,蒂长 7cm— 12 cm。结果 :2例皮瓣出现完全坏死 ,16例愈合良好 ,其中 3例皮缘小部分坏死 ,剪切坏死边缘后缝合 ,愈合良好。 5例皮瓣出现瘀血肿胀 ,有散在水泡 ,不影响愈合。结论 :小腿后侧逆行岛状筋膜皮瓣血运丰富可靠 ,切取操作简便 ,是修复胫前下1/3段、踝周。

Objective:To investigate the characteristics of clinical application of distally based island fascial flap on the posterior calf.Methods The flaps were used in 18 cases to repair the solf tissue defect in heel,arond ankle ,low anterior tibial and tendon.The Largest flap size was 12cm×9cm,the smallest 4cm×3.5cm.Results 16cases survived satisfactorily,2 cases presented completely necrosis.Conclusions This falp is easy to elevated and its blood supply is reliable.It is an ideal flap for repairing the solf tissue defeet in low 1/3 part of anterior tibial,around the heel and the ankle.
