肌瓣移植的临床应用及组织学观察

Clinical application and histological observation of the transferred muscle flap

目的 为了解无皮肤的肌瓣转移修复口腔内恶性肿瘤切除后软组织缺损的愈合过程。方法 用游离或带蒂胸大肌瓣转移修复 4例口腔颌面部恶性肿瘤切除后软组织缺损的患者 ,并对肌瓣移植后的临床愈合过程进行了组织学观察。结果 裸露于口腔内肌瓣 8～ 12周后完全上皮化 ,平均术后 10个月到一年随访观察 ,效果良好。结论 单纯肌瓣移植不仅为口腔内软组织缺损的重建提供一安全可靠的方法 。

Objective In order to observe the healing process of the naked muscle flap used for reconstruction of the intraoral soft-tissue defect.Methods Four patients with the defects in the oral cavity and maxillofacial region were reconstructed by using pectoralis major flap as a pedicle graft or free graft.The clinical healing process were observed by light microscopy.Results The uncovered muscle flap in the mouth underwent eventulal epithelialization by oral mucosa after 8～12 weeks.All cases were followed up for mean 10 months to 1 year after surgery.A good result had been obtained.Conclusion The muscle flap decreased malformation of skin defects.It was a safe and reliable method for the oral soft-tissue defect reparing.
