摘要
目的 探讨三维CT血管造影 (3D CTA)对脑梗死的诊断作用。方法 应用ToshibaAquilion型多层螺旋CT扫描仪对 2 6例脑梗死病人进行 3D CTA检查 ,分别对大脑前动脉、大脑中动脉、大脑后动脉和椎 基底动脉系统进行检测并评估。结果 发现大脑中动脉闭塞或狭窄 17例 ,大脑后动脉闭塞或狭窄 2例 ,椎动脉狭窄 1例。 6例未见血管异常 ,其中 5例为多发性腔隙梗死。全部 2 6例病人中 ,只见到 1例动脉内壁不光滑 ,未见到斑块、钙化、溃疡及血栓形成。结论 脑梗死的病因可能以栓塞为多见 ;3D CTA具有灵活的三维影像后处理功能 ,能提供脑血管病的多方面信息 ,是诊断脑梗死的重要工具之一。
Objective To study the role of three-dimensional CT angiography (3D-CTA) in diagnosing cerebral infarction. Methods 3D-CTA were used to detected 26 patients with cerebral infarction. The anterior cerebral artery (ACA)? middle cerebral artery (MCA)?posterior cerebral artery (PCA) and vertebrobasilar artery were studied and evaluated respectively.Results 17 MCA occlusion or stenosis, and 2 PCA and 1 vertebral artery occlusion or stenosis were found. There were not abnormal vessels in 6 cases, 5 of them were lacunar infarction. Large vessel occlusion and relationship between the Willis circle and surrounding structures were clearly and were displayed surely. Thrombus and calcification were not found on the vessel wall by fly-through. The artery wall was almost smooth.Conclusion Embolism might be the major cause for most of the cerebral infarction; 3D-CTA has flexible after-treatment function of 3D image and can provide multi-aspects diagnostic information for cerebraovascular disease (CVD). 3D-CTA is favorable for locating the cause of CVD, and it is an important tool for the diagnosis of cerebral infaction.
