新生儿缺氧缺血性脑病血糖、血IGF-1水平变化及临床意义 被引量:4

THE CHANGE AND CLINICAL SIGNIFICANCE OF BLOOD GLUCOSE AND SERUM INSULIN-LIKE GROWTH FACTOR-1 IN NEONATAL HYPOXEMIC-ISCHEMIC ENCEPHALOPATHY

摘要 研究新生儿缺氧缺血性脑病(HIE)血糖、血中胰岛素样生长因子-1(IGF-1)的变化,为临床治疗HIE提供参考依据,并探讨对IGF-1在HIE发病机制中的作用。用放免法测定了48例HIE患儿生后第一天、第七天及40例正常新生儿生后第一天空腹外周静脉血IGF-1,同时用微量血糖仪测微量血糖。结果显示,与正常新生儿相比HIE患儿血糖显著升高(t=2.613 P<0.05),IGF-1则显著减低(t=4.933 P<0.05);不同程度的HIE血糖无明显变化(第1d F=0.714 P>0.05第7 d F=1.743P>0.05),IGF-1下降明显(第1 d F=9.004 P<0.05第7 d F=5.910 P<0.05);经直线相关分析证实HIE患儿血糖和IGF-1呈负相关(r=-0.353 P<0.05)。可以确认,IGF-1与HIE程度有关,在其发病机制中可能具有重要作用。 To observe the blood glucose and serum Insulin-like Growth Factor-1 (IGF-1) Ievel in Neonatal Hy-poxemic-ischemic Encephalopathy (HIE) and the effect of IGF-1 on pathogenesis of HIE, to find evi-dence for therapv. Authors prospectivelv observed the variation of IGF-1 in peripheral blood at the time of l and 7 days after birth in HIE and healthv controls by using radioinimunoassay. Blood glucose was de-tected by glucosemeter at the same time. Results showed that HIE group had higher and lower IGF-1 level than healthy control (P < 0.05) . Blood glucose was same and Serum IGF-1 dropped obviousl in different degree of HIE. There was negalive nelation between blood glucose Ievel and IGF-1 in neonatal HIE, r = - 0.353, P < 0.05. Serum IGF-1 dropped plaved an important role in protecting centra! neuron systen. We conclude that IGF-1 has some relation with the ceverity of HIE, and it may play come role in the de-velopment of HIE.
