肾脂肪囊内注射黄芪对内毒素休克所致急性肾衰竭大鼠的防治作用

Therapeutic effects of astragalus injected into renal adipose capsule on acute renal failure induced by endotoxic shock in rats

目的 :观察肾脂肪囊内应用黄芪注射液对内毒素休克所致急性肾衰竭的影响。方法 :雄性 SD大鼠随机分为 4组 :对照组、大肠杆菌内毒素脂多糖 (LPS)组、黄芪静脉给药组及黄芪肾脂肪囊给药组 ;测定各组相应时间点的平均动脉压 (MAP)、肌酐清除率 (Ccr)以及尿量 (UV) ,并观察各组大鼠光镜下肾小管的病理改变。结果 :LPS组大鼠 MAP、Ccr及 UV显著降低 ,肾脏病理显示肾小管上皮细胞混浊肿胀 ;静脉给药组上述指标轻微改善 ,与 LPS组相比仅 UV有显著差异 ,肾脏病理改善较轻 ;肾脂肪囊给药组 Ccr及UV改善明显 ,尤以 12 h和 16 h为著。结论 :应用黄芪有利于纠正内毒素休克 ,改善肾功能 ,减轻肾脏病理损害。相对于静脉给药 ,对肾脏保护效果更明显 ,有可能成为临床治疗内毒素休克及急性肾衰竭的一个新手段和给药途径。

Objective:To observe the effects of Astragalus injected into renal adipose capsule on acute renal failure (ARF) induced by endotoxic shock.Methods:SD rats were randomly divided into 4 groups: the control group, Lipopolysaccharide (LPS) group, group for intravenous injection of Astragalus, and group for renal adipose capsule injection of Astragalus. The changes of mean arterial pressure (MAP), creatinine clearance rate (Ccr) and urinary volume (UV) were observed at different time points. The morphological changes of kidney were observed under the light microscope. Results:Compared with those in control group, the MAP, Ccr and UV were significantly declined in LPS group. In LPS group, renal tubule epithelial cells showed cloudy swelling under the light microscope. There was a slight improvement on MAP, Ccr and UV in group for intravenous injection of Astragalus. The structural changes were attenuated. There was also a significant improvement on Ccr and UV in the group for renal adipose capsule injection of Astragalus. The effect of Astragalus injected into renal adipose capsule was obviously better than that of the intravenous one at 12 and 16 hours after injection (P<0.05).Conclusion:Astragalus could correct endotoxic shock, promote recovery of renal function and improve the histomorphological injury of kidney. Renal adipose capsule injection of Astragalus may be a potential therapeutic route in the management of endotoxic shock and acute renal failure.
